Please bring the following items to the Parent Meeting:
- Copy of Birth Certificate
- Wallet size photo
- *LSR-Medical Form (instructions below)
- *LSR-Waiver and Release Form (instructions below)
- Magic Sports Player/Parent Agreement Form (at the
bottom)
- Uniform Form (at the Bottom)
Please print the Magic Volleyball Agreement Form (below) look them over carefully, sign in appropriated areas and bring to the meeting. We will be going over the contracts at the meeting if anyone has questions at that time I will be
happy to address them.
We will be trying on and ordering Uniforms at the Parent Meeting. Please print out the
Uniform Form (below) so that we will have the player info as well as so that you will know what extra items will be available. We will have samples for the
girls to try-on.

*Registration will be done online this year. The instructions are as follows:
- Go to the Lone Star Region website www.lsvolleyball.org.
- Click on the Registration link up on the top right of the screen. In the middle of the screen will be 2 options. One is for a players who played club in the
2010-2011 season and the other is for a new player. Please select the
appropriate icon.
- The next Page will ask for your name and email address. Fill out that info and they will email you a password.
- After receiving your new password go back to the Lone Star website and login and fill out the online registration.
- You will need to fill out and bring the Medical Form as well as the Waiver and Release of Liability to the Parent Meeting.
- It will tell you that you have a fee due for
registration. Please ignore this. Your fees cover this
registration.
***Very important. You only have one opportunity to select the club that you will be playing for. Please enter Magic Volleyball.
